To re-order contacts or for more information about our services, please complete the form below. Please keep in mind that contact lens prescriptions are only valid for 1 year. You will need a current, valid prescription before you order more contacts. If you have not had your prescription updated please contact the office to schedule an appointment with one of our optometrists. For all Non-Disposable Contact Wearers (GP/RGP) please contact the office to re-order your lenses.
